Adapting PE.  Neither the SENCo at my daughter's school, nor her physio, seems to know where to start, but I've recently seen snippets from the CD-Rom issued to schools by the DCSF on Implementing the DDA in Schools and now know that there's heaps of ideas out there.  
Have you checked out the Inclusive Solutions website?  Or www.worldofinclusion.com?  I recently attended a day's training by Richard Rieser and he and his team are only too willing to train or advise teaching staff on inclusive practice.
Otherwise I'm reading Seeing the Charade - What We Need to Do and Undo to Make Friendships Happen.  V American, but makes a lot of sense.  I can now appreciate how some practices (that are supposed to help children with SEN access the curriculum) can be counter-productive; set the child apart and perpetuate low self-esteem.  It's scary.  Could go on ...
